The Australian Transport Safety Bureau (ATSB) has been unable to identify the cause of a fatal crash of a Cessna 421C in Western Australia in August 2004 which resulted in the deaths of the pilot and the passenger. The investigation has discounted anomalies or failures in aircraft systems, lateral imbalance, pilot medical issues and fuel factors, with the ATSB adding that the pilot might have been momentarily distracted during the take-off, resulting in the left bank.
