Orbital Science says, that the strongest evidence of the cause of the Pegasus XL launch failure on 22 June, is that one of three nozzle guides on the second stage, which are situated 120° apart and help to guide the interstage structure off the vehicle during first/second stage separation, was installed improperly. The interstage remained on the nozzle, constricting its steering capability.
