Thanks to all of the readers who suggested an explanation this week.  Unfortunately nobody guessed correctly. 

Last week’s image shows a Boeing Delta rocket carrying the European Space Agency's first experimental communications satallite, OTS-1, lifting off from Canaveral Air Force Station only to be destroyed 103 seconds later. 14 September 1977.
