Orbital Sciences says that the most likely cause of the Pegasus XL launch failure on 22 June is that one of three nozzle guides on the second stage, which help to guide the inter-stage structure off the vehicle during first/second stage separation, was installed improperly. The inter-stage remained on the nozzle, constricting its steering capability.
