A GARUDA INDONESIA Airlines McDonnell Douglas DC-10-30 overran the runway at Fukuoka, southern Japan, shortly after midday local time on 13 June, following the captain's late decision to abort the take-off. Of the 260 passengers and 15 crew on the flight, bound for Jakarta, Indonesia, via Denpasar, Bali, three passengers died and 108 were injured.

Early reports indicate that the captain's decision was based on a mechanical failure in the No 2 (tail) General Electric CF6-50C engine, possibly because of secondary effects from the failure. Japan Air System has already ordered checks on similar engines used on its DC-10s.

The aircraft had just become airborne from the airport's 2,800m (9,100ft)-long runway, when, say passengers, two bangs were heard before the DC-10 was set down again on the runway.

It overran the runway by about 400m on flat ground, broke through a perimeter fence, and came to rest substantially intact, but with the gear collapsed. Fire eventually destroyed the fuselage aft of the wing. There is a large hole on the starboard side, of the No 2 engine cowl, near the compressors and damage to the starboard stabiliser leading edge.
